Billing: Payment must be by credit card (VISA, Mastercard, Discover Card, or American Express accepted) or purchase order. In order to protect your financial information, we ask you do not fax or e-mail your credit card information. Please provide your daytime phone number on the form and a representative from the Pittsburgh Poison Center will call you regarding your billing information.
