Applicants to this program should complete and submit the fellowship application form to Shelley Williams, MD, (EEG/Epilepsy) or David Lacomis, MD, (EMG), along with three letters of recommendation and documentation of completion of USMLE and (if a foreign medical graduate) ECFMG examinations.
