Payment/Insurance
Please bring your child's insurance identification information or your child's Department of Medical Assistance Access card with you. Prior authorization for each visit must be obtained if you have coverage through an HMO (Health Maintenance Organization) or PPO (Preferred Provider Organization). If proper authorization is not obtained prior to your child's visit, you will be responsible for payment. If your plan requires a co-payment for an office visit, please bring cash or a check in the appropriate amount.
