To help the staff evaluate your child, please bring all of the following items that apply to your child when you come for your child's first visit:
- Copies of medical records
- Copies of past hearing tests
- Copies of laboratory results
- All radiology tests/studies
- Referral from your child's doctor
Photocopies of the above information will be made, and all originals will be returned to you, your child's doctor or the lending hospital.
