Diabetes
Description of Services
For more than three decades, endocrinologists at Children’s Hospital have played a major role in the care of children with diabetes and all types of hormone-related disorders. Children’s has one of the largest pediatric diabetes clinics in North America. The program is a leader in both clinical care and research in many of the issues surrounding childhood diabetes mellitus and provides consultative services and long-term management of children with diabetes mellitus. Physicians, dietitians, certified diabetes nurse educators, nurse practitioners and social workers staff the program. When needed, staff facilitates referrals for psychiatric care.
Referral Requirements
Referrals may be made by telephone or mail by primary care physicians, school nurses, nutritionists or other Children’s Hospital specialty services. All newly diagnosed patients should be referred by direct contact with the endocrinologist on call. Referrals should be accompanied by related patient records and growth charts. Referrals for patients enrolled in managed care insurance plans also require authorization from the primary care physician and sometimes from the insurance provider. All necessary referral and authorization forms must be received before the patient’s visit and include separate authorization for each physician, diabetes nurse educator, dietitian, laboratory and X-ray service.
