Camp Application Forms

June 16th to 20th, 2009

Camper Application (PDF) - Due May 1, 2009
Campers must be 8 years old to 15 years old on 1st day of camp.

Counselor Application - Due Feb. 15, 2009
Counselors must be 18 by first day of camp and be a current of former Children's Hospital of Pittsburgh of UPMC patient.

Counselor In Training - Due Feb. 15, 2009
Counselors in training must be 16 or 17 at start of camp and must be current of former Children's Hospital of Pittsburgh patient.

Nursing/Medical Staff Application (PDF)
Nursing and Medical staff must be Children's employees.

Volunteer (Non-Medical) Staff (PDF)
Please contact 412-692-6054 if you are not a Children's employee.

Prior to acceptance of application, all volunteers and counselors who are 18 years of age or older must contact the Volunteer office and complete the volunteer application process if they are not an employee of Children's Hospital. Volunteer Services can be reached at 412-692-5185.
