What to Bring

To help the staff evaluate your child, please bring all of the following items that apply to your child when you come for your child's first visit:

• Copies of medical records
• Copies of past hearing tests
• Copies of laboratory results
• All radiology tests/studies
• Referral from your child's doctor

Photocopies of the above information will be made, and all originals will be returned to you, your child's doctor or the lending hospital.
