Planning Your First Visit

Adult Congenital Heart Disease Center

In order to provide the best care possible for our patients, we require two weeks to collect and review your charts and prepare for your office visit.

Patients have two options when planning your first visit to the Adult Congential Heart Disease Center.  Appointments can be made at either Children's Hospital of Pittsburgh of UPMC or UPMC Presbyterian. Choose the location that is most convenient for you - the expertise is the same.

Medical Records

Please have your cardiologist or referring physician mail or fax all cardiac-related medical records. In addition, please provide a copy of an echocardiogram performed within one year (or most recent test) on a CD, so it can be reveiwed by our adult congenital heart disease experts prior to your visit.

The following records are also necessary for your visit:

  • Cardiology progress notes
  • Echocardiogram report (including stress echo and transesophogeal echo reports)
  • Cardiac MRI/CT
  • Electrocardiographms (ECG)
  • Cardiac catheterizations
  • Surgical notes
  • Recent labs
