My Pain Wall
Sometimes it’s helpful for parents to keep track of a child's pain. Medicines and daily activities can cause changes, making the pain get better or worse. These changes and patterns in pain can be important to know and understand, especially for the nurses and physicians who care for your child.
Download the “My Pain Wall” assessment form (PDF) to track your child's daily pain experience by using cartoon faces and words to describe pain levels. Also, you may want to write notes to record details about your child’s pain.
