The Division of Adolescent and Young Adult Medicine systematically coordinates care to identify, assess and counsel individuals with complex medical and psychosocial needs, and assists in the transitioning of adolescents and young adults into adult healthcare. This patient centered approach integrates services, care, and support for persons with comorbid health, mental health and substance abuse problems including seamless linkage to relevant resources and additional supports in their community.
Integrating mental health, substance abuse, and primary care services produces the best outcomes and proves the most effective approach to caring for people with multiple healthcare needs.
Our staff is guided by the Integrated Healthcare Model which supports behavioral health needs of children, adolescents, and transitional-age persons 10 to 25 years old who are at risk for mental illness, substance abuse, and suicide, and among the least likely to seek continuous help. Our team members encompass multiple disciplines and include psychiatrists, psychologists, and behavior specialists.
