Our dedicated nurse practitioners will help with a smooth transition and be a vital member of your ACHD care team.

We provide a transitional care plan to guide you into the next adult phase of your life.

Your complete congenital heart disease care plan will include details about your follow-up medical needs and guidance on topics such as:

  • Fertility, birth control, pregnancy, family planning
  • Psychosocial issues
  • Career counseling
  • Sporting activities
  • Insurance issues
  • Travel
  • Other future plans
