Important Documentation

Before scheduling an on-site visit, families will be mailed an information packet and may be asked to complete various questionnaires, as well as consent forms that allow the CDU to obtain information from other sources. Families also may be asked to forward copies of their child’s medical, birth and school records, including achievement/intelligence test results.
