Camp Application Forms

June 11–15, 2013

Heart Camp registration closed on Friday May 3rd. Please email the Camp Coordinator Keith McIntire at keith.mcintire@chp.edu to see if there are any available spaces.

Campers must be 8 years old to 15 years old on 1st day of camp.

Nursing/Medical Staff Application
Nursing and Medical staff must be Children's employees.

Volunteer (Non-Medical) Staff
Please contact 412-692-6054 if you are not a Children's employee.

Prior to acceptance of application, all volunteers and counselors who are 18 years of age or older must contact the Volunteer office and complete the volunteer application process if they are not an employee of Children's Hospital. Volunteer Services can be reached at 412-692-5185.
