Medical Assistance Transportation Program (MATP) — The Pennsylvania Department of Human Services offers transportation options available in your county.

Office of Vocational Rehabilitation (OVR) — The Pennsylvania Department of Labor & Industry offers career services for people with disabilities and a directory of contacts by county.
Voices for Independence — Provides independent living supports and services.
Personal Health Record (PHR) (PDF) — is a tool that you can use to collect, track, and share past and current information about your health or the health of someone in your care. A PHR gives you a home to maintain and manage your health information in a private, secure, and confidential environment.
