The LeFort 1 osteotomy (oss-tee-OT-uh-mee) is a type of orthognathic surgery — another term for jaw surgery. It is used to correct deformities and misalignments in the midface. The LeFort 1 is a common and safe procedure used by surgeons to realign the upper jaw and teeth to improve both appearance and function. Repositioning the upper jaw can have a positive impact on a child’s confidence and quality of life. A LeFort 1 can be performed alone or in combination with a lower jaw (bilateral sagittal split osteotomy) or chin surgery.

Fast Facts about LeFort 1 Osteotomy
- LeFort 1 is the most common procedure for treating jaw misalignment, cleft lip or palate, and upper jaw fractures.
- It allows the upper jaw to be repositioned to reshape the face and improve the bite.
- The LeFort 1 usually requires orthodontic work before and after the procedure.
- The procedure is typically done when a child’s bones are mature. For girls, that is generally between ages 14 and 16, or 2 years after their first menstrual period. For boys and young men, that’s normally between ages 16 and 18.
- During a LeFort 1 osteotomy, the upper jaw (maxilla) is separated from the rest of the facial skeleton so it can be repositioned.
- Once separated, the upper jaw can be moved up, down, forward, backward, tilted, or turned. There can also be a combination of these movements.
- The LeFort 1 surgery is performed entirely inside your child’s mouth so there will be no visible scar on the face.
- Your child will be given medicines to prevent pain during their surgery. It will be based on what the anesthesiologist (AN-es-theez-e-ol-o-gist) decides is best for your child.
- This surgery takes 2 to 3 hours.
- Children who undergo a LeFort 1 procedure typically spend 1 night in the hospital.
