Referral Requirements
Referrals may be received from any source. Self-referrals are accepted, although a physician’s referral and prescription is required for billing insurance companies. Authorization from the insurance provider and primary care physician are often required, especially for those enrolled in managed care insurance plans. All necessary referral, prescription, and authorization forms must be received before the patient’s visit.
