The transition program at the Adult Congenital Heart Disease (ACHD) Center is one of a kind.
This joint program of UPMC Children's Hospital of Pittsburgh and UPMC Presbyterian promotes lifelong care for people with congenital heart disease.
The shift from childhood to adult congenital heart disease care can be overwhelming and confusing. Our ACHD Center is here to help ease the stress and confusion that sometimes come with a change in care.
Our dedicated nurse practitioners will help with a smooth transition and be a vital member of your ACHD care team.
We provide a transitional care plan to guide you into the next adult phase of your life.
